  1. Determine the severity of the problem (eg. how bad is it?)
  2. Duration (eg. How long has it been going on and how was the situation handled, if handled at all the first, second, or third time around?)
  3. Outside sources (eg. Drop off kids off school, that's why late)
  4. Degree of Socialization (eg. Was the employee informed of rules and regulations at the workplace?)
